Advanced Practice Registered Nurses / Physician Assistants
Carle Health is seeking an experienced Nurse Practitioner to join our multidisciplinary team in providing psychiatric care as a consultant to a large medical/surgical inpatient service.
Practice Details Include:
Serves as a member of the Psychiatric Consultation Liaison Service at Carle Foundation Hospital.
Collaborates with Consult Liaison team to facilitate the management of complicated mental health cases.
Assesses patients in consultation on medical/surgical units. Performs and documents history and mental status examination based on age and history on complex mental health and addiction issues. There will likely be an expansion into the Emergency Department of this service.
Assesses the impact of an acute and/or chronic illness on the individual’s: health status (physical and mental), functional status, including activity and mobility, nutritional status, sleep and rest patterns, quality of life, family, social, and educational relationships
Assesses patient/family bio/psycho/social needs considering physical and age-specific needs, recognizing the dynamic nature of the patient’s condition, and provides psychosocial support to patient and family members, in coordination with the primary care team and case management.
Provides comprehensive assessment, plan development, implementation of plan of care and evaluation of effectiveness of interventions with revisions as necessary.
Incorporates health promotion, health protection and injury prevention measures into the plan of care within the context of the complex acute and chronic illness
Prescribes medication in accordance with practice agreement with the collaborating physician; prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
Monitors medication and initiates appropriate therapies for certain conditions and changes medications according to variations in a patient's response within established protocols.
Educates patients/families about self-management of acute and chronic illness with sensitivity to the patient's learning ability and cultural and ethnic background. Collaborates with health care team in ensuring that patient/family needs are met
Assists in facilitating the patient's transition between and within health care settings, such as admitting, transferring, and discharging patients; coordinates discharge plan and follow-up of patients in collaboration with the physician
Assist in education of residents, medical students, Nurse Practitioner students, and Physician Assistant students.
Documentation consistent with established policy and procedures to create a permanent patient record
2+ years of experience as NP in acute adult/child inpatient psychiatric unit, background and knowledge of psychiatric and medical-surgical illnesses and treatments required.
Be able to work with child, adolescent, adult and geriatric patients on medical, surgical, observation units and the staff assigned to the unit.
The NP demonstrates an advanced level of medical and nursing knowledge, clinical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation.
Cross training in the outpatient setting is possible.
Current IL APN License with certificate of fitness to prescribe.
Certification as a Psychiatric Mental Health Nurse Practitioner.
